Are we actually living in a simulation?

Nobody knows. The simulation argument is a philosophical thought experiment: if advanced civilisations could run many realistic simulated worlds, simulated minds might outnumber real ones. It hasn't been proven or disproven, and this test doesn't claim either way.

What does 'player' vs 'NPC' mean here?

It's a game metaphor for agency. An 'NPC' lets defaults run their life; a 'player' chooses goals, takes risks they can survive and learns from feedback. Anyone can move from one to the other with small, deliberate habits.

    Is this test scientific?

No. It's a quick self-reflection based on your habits around goals, risk, practice and attention, made to be fun and useful, not a psychological assessment.
